Returning Home & Follow-Up Checklist

The trip home is part of the treatment. Leaving with complete records and a clear follow-up plan is what turns a successful visit into lasting care.

Before leaving Cincinnati

  • Visit or discharge summary in hand — and understood
  • Copies of new records, reports, and imaging for your local team
  • Every medication clear: name, dose, schedule, and how to get it at home
  • Follow-up plan confirmed: appointments, remote visits, tests to do locally
  • Warning signs explained: what to watch for and what to do
  • Flight clearance confirmed with the care team, if relevant

First weeks at home

  • Appointment with your local physician, records delivered
  • Local pharmacy plan for ongoing medications
  • Remote follow-up visits scheduled, if the provider offers them
  • Questions routed through the provider’s official channels — your coordinator can help

Closing the case

  • A complete written record of the journey kept by the family
  • Feedback shared with your coordinator — it improves the service for the next family
  • Data-retention preferences confirmed — ask what we keep and what we delete

This checklist is general information, not medical, legal, or immigration advice. Requirements vary by hospital and country — your coordinator confirms the specifics for your case.
