Checklist
Visa-Support Document Checklist
One set of truths first: the hospital’s letter supports the application, the consular officer decides, and every traveler may need their own application. Now the list.
For every traveler
- Valid passport (check the expiry date generously)
- Completed application form and fee receipt
- Photo meeting official requirements
- Evidence of ties to home — work, study, property, family
- Previous visas or travel history, if any
For the medical purpose
- Hospital acceptance or appointment letter
- Statement from the local physician about the condition, where appropriate
- Hospital cost estimate
- Proof of funding for care and stay
- Support letter from Cincy Kids Bridge describing arranged coordination (supplement, not substitute)
Rules that protect you
- Apply as early as your documents allow — interview waits vary by country
- Answer interview questions plainly and truthfully
- Buy no nonrefundable travel before the visas are approved
- Complex immigration situations deserve a qualified immigration professional — we do not give legal advice
This checklist is general information, not medical, legal, or immigration advice. Requirements vary by hospital and country — your coordinator confirms the specifics for your case.
